Referees

Referees under the age of 18 need to provide NYSA with:
1) Proof of Birth ->
2) Parent/School Authorization document ->
3) W9 form ->

Referees over the age of 18 need to provide NYSA with:
1) W9 form -> -- provide just once.

Players

All new players need to provide proof of birth -> to complete their initial NYSA registration. The proof of birth can be faxed -> , emailed to office@northshoresoccer.org, mail be mailed to the NYSA office -> , or it can be scanned and uploaded to the NYSA website by clicking on Member, Paperwork, Upload -> .
